The Chenango & Delaware Counties Systems of Care is a team made up of local community providers, consumers and their family members that have a goal of linking individuals with resources and supports in Chenango and delaware Counties. Whether it is mental health, substance abuse, housing, transportation, or crisis services our aim is to help ensure all residents have equal access to information on resources to meet their unique needs.

Mission
Chenango & Delaware Counties SOC is a team made up of a dedicated, cross-systems group of individuals, families, and community providers, whose core values are building connections to strengthen the well-being of individuals and communities using a family-driven and youth-guided approach.
Vision
Our hope is for all families and youth to have knowledge and access to the most appropriate services in the counties. The SOC process will ensure providers are able to focus on the needs of each family and collaborate with each other to reduce barriers and increase access to these services.
